Golden LEAF requires an interim report to be submitted six (6) months from the grant award date, then every six (6) months thereafter for as long as the grant is active. A final report is due within 60 days of the end date of the grant.
The Golden LEAF Foundation Grantee Agreement requires that grantees immediately notify us of any proposed changes to the funded project, including project goals, objectives, timelines, or the grant budget. Any changes to a grant must be approved by Golden LEAF prior to implementation or expenditure of the modified components.
Following award, grantees attend a Grant Management Workshop for training on how to use the customized report form. Forms will be provided to grantees following an award. Below you’ll find a link to an example Golden LEAF comprehensive report form. This link is for information only. Please contact your Program Officer or [email protected] if you are unable to locate the reporting form created for your project.
Most Golden LEAF grants report using a comprehensive report form workbook for all reports, disbursement requests, and modification requests:
Some older grants may use different reporting forms. If your grant does not use the comprehensive reporting form or if you are not certain which form to use, please contact your Program Officer.
ACH Authorization Form:
Golden LEAF now makes payment using the ACH system. Grantees will need to complete an ACH Authorization Form and mail or fax back to Golden LEAF. Due to the sensitive nature of this information, we do not recommend emailing this form.

If I receive a grant award, when will funds be available?
Availability of funds depends on the conditions that may be applicable to a grant. Once all conditions on the release of funds have been met and Golden LEAF receives a disbursement request, funds are usually available within 30 days.
If a grant is awarded, what are the reporting requirements?
Golden LEAF’s standard reporting schedule requires an interim report to be submitted six (6) months from the award date, then every six (6) months thereafter for as long as the project is active. A final report is due within 60 days after the end date of the grant. Golden LEAF also frequently requires reporting after the end of the term, typically annually.
What if I need a budget or project modification?
Golden LEAF requires that grantees immediately notify us of any proposed changes to the funded project, including project goals, objectives, timelines, or budget. Any changes must be approved by Golden LEAF prior to implementation of the proposed change or expenditure of grant funds on modified budget line items. Project and budget modification forms are included in the current comprehensive project reporting form.
