This grant funded Roanoke Chowan Community Health Center to establish a rural health clinic in Woodland, NC in a former bank building. The new clinic replaced the only existing primary care office in the Woodland community, which closed after 62 years. The new clinic worked with the retiring physician from that practice to transition his patients, ensuring continuity of care. The new Woodland clinic housed four full-time equivalent and two part-time positions and served 500 patients. Golden LEAF funds were used for construction and renovation.
