Description:

The purpose of this grant is to assist Vidant Duplin Hospital, in collaboration with the Duplin County Health Coalition, with a new effort that will implement a telehealth program that will help to provide case management to patients with chronic disease and reduce unnecessary Medicaid and Medicare expenses. In addition to the in-home telehealth component, the project also calls for the placement of four remote telehealth kiosks throughout Duplin County in order to create self-entry points for other patients. Golden LEAF funds will be spent for personnel and equipment.
